Estradiol: Cream vs Capsule, Explained

Both estradiol body cream and estradiol capsules deliver the same bioidentical hormone systemically. The difference is in how the hormone reaches your bloodstream — and that difference matters clinically.

Transdermal cream

Applied to the skin, estradiol cream absorbs directly into the bloodstream and bypasses first-pass liver metabolism. Because it skips the liver, transdermal delivery may be associated with a more favorable cardiovascular and clotting profile.

Oral capsule

Taken orally, estradiol capsules deliver consistent, measurable dosing with no topical application. Hormone is processed through the digestive system and distributed throughout the body.

Local vaginal cream — different role

Vaginal estradiol cream works locally rather than systemically. It directly restores vaginal tissue health — moisture, elasticity, and comfort — and is especially helpful for genitourinary symptoms of menopause that don't ease with time.
